If you would like your child to be a Palmetto Parent Birthday club contestant, complete this form along with a recent personal photograph of your child (one that was not taken professionally).
Entries must be received by the 5th of the month prior to the month the birthday occurs. Child must be under 12 years to win. By entering, contestants and their parents (or guardians) release the photo to Palmetto Parent with no further obligation to Integated Media Publishing and give consent to Palmetto Parent to reproduce and publish any photographs submitted whether or not the entry is chosen as a winner. All entries must be approved with consent box checked by the parent/guardian. Employees and family members of Integrated Media Publishing and contest sponsors are not eligible to win. Winners will be selected randomly from all entries received and must claim prizes and book party within 30 days.
